What the Science Actually Says About Reishi

Reishi has been revered for two thousand years. But reverence isn't evidence — so what happens when this ancient mushroom meets the modern clinical trial?
The honest answer is more interesting than the marketing.
A mushroom under the microscope
Reishi (Ganoderma lucidum) has been studied more than almost any other medicinal mushroom. Hundreds of laboratory and animal studies have explored its two great compound families — the beta-glucans and the triterpenes — and found real, measurable activity: immune signalling, antioxidant effects, anti-inflammatory action.
But laboratory findings and human results are different things. And when we look at what's been tested in actual people, the picture becomes more modest.
What the human trials show
The most rigorous recent synthesis pooled seventeen randomised controlled trials covering 971 participants. It found that Reishi supplementation produced small but statistically significant improvements in a handful of measures: body mass index, resting heart rate, creatinine (a marker of kidney function), and glutathione peroxidase — one of the body's own antioxidant enzymes.
That last one is worth pausing on. It suggests Reishi may support the body's internal antioxidant machinery, rather than simply supplying antioxidants from outside. That's a mechanism traditional herbalists would recognise: not doing the work for the body, but helping the body do its own.
Equally telling is what the review didn't find. Across those trials, there were no significant effects on blood pressure, fasting blood sugar, cholesterol, inflammatory markers, or liver enzymes.
And the researchers were candid about their confidence: the overall quality of the evidence was rated very low. Most Reishi trials have been small, short, and used wildly different preparations — different species, different extraction methods, different doses. It's difficult to compare a hot-water extract to an alcohol extract to a spore powder and call them the same intervention.

The safety picture
Here, the evidence is more settled. Placebo-controlled trials in healthy adults have found Reishi well tolerated, with no meaningful changes to kidney or liver function over twelve weeks of use. For a supplement taken daily and long-term, that matters.
The provenance problem
There's one finding that should give every Reishi buyer pause. When researchers genetically tested commercial reishi products, they found that many didn't contain the species on the label at all.
This is the quiet scandal of the functional mushroom industry — and it's a large part of why we grow our own. We know exactly what's in the bottle because we watched it fruit.
Where this leaves us
Reishi is not a cure. The clinical evidence is early, uneven, and honestly reported as such by the people who gathered it. What we have is two thousand years of traditional use, a rich and genuinely active chemistry, a reassuring safety profile, and a small but growing body of human research that hints at gentle, supportive effects.
That's not a miracle. It's something better: a plant ally, honestly described.
